Some of the records included infor-
mation about a confined number of
beneficiaries of the Walmart Inc. Asso-
ciates' Health and Welfare Plan. Nei-
ther Walmart Inc.'s, nor the Walmart
Inc. Associates' Health and Welfare
Plan's information systems were af-
fected by this incident. Walmart Inc.

dent response services to the Walmart
Inc. Associates' Health and Welfare
Plan and immediately began investi-
gating to determine whether any per-
sonal information handled by the busi-
ness associate was affected.
The investigation revealed the infor-
mation affected appears to be an el-
igibility file that included some bene-
ficiary names, beneficiary dependent
names, addresses, dates of birth, and
health plan subscriber identification
numbers.
The Walmart Inc. Associates' Health
and Welfare Plan, and Walmart Inc.,

and are fully committed to protecting
the privacy and security of protected
health information. Although neither
Walmart's nor the Walmart Inc. Associ-
ate Health and Welfare Plan's systems
were impacted, we have reviewed the
business associate's security practic-
es surrounding this event.
The Walmart Inc. Associates' Health
and Welfare Plan is sending individual
notices to affected beneficiaries, and
the personal representatives of ben-
eficiary dependents, regarding this
incident. If you have any questions,
we encourage you to contact our ded-
icated call center at 866-731-3112.

efits and other information pertaining
to you and always be cautious about
requests to provide any personal or
financial information over the phone,
by text or by email. Be sure to always
verify the source of any request for
such information.
The Walmart Inc. Associates' Health
and Welfare Plan places great value
in the trust of beneficiaries and takes
this matter very seriously. We are fully
